Carers Pass

Southend Carers and Mid and South Essex NHS Foundation Trust, are committed to working in partnership with patients and Carers to enhance the treatment and care received when they visit Southend University Hospital. 

Many carers visiting the Hospital, often ask what community help and support is available to them once the patient leaves hospital.  So we have joined forces to support both Carer identification via the Carer’s ID Pass – a new innovative photo ID card, and to also create a smoother pathway for Carers to connect to community support services.

Over the next few months, the Carers Passport will include the offer of the Carer’s ID Pass, and be offered (for the purposes of the pilot) to those Carers visiting the Department of Medicine for the Elderly (Hockley, Chalkwell & Balmoral wards). 

The Carer’s ID Pass will;
1. help identification and recognition whilst visiting Southend University Hospital
2. ensure Carers are connected to essential carers information, advice and community services
3. help Carers to access additional support when visiting the hospital

And best of all the Carers Pass card is FREE!

If you are a Carer visiting the Department of Medicine for the Elderly (Hockley, Chalkwell & Balmoral wards), speak to a member of the ward staff to discuss the Carers Passport and Carers Pass.
