A carer is someone who provides unpaid care and support to a friend, relative or neighbour who is elderly or has a condition such as a disability, illness, mental health condition or a drug or alcohol problem.

Being a carer is not easy – but you are not alone and we are here to support you.

Please note that due to Coronavirus, we are working remotely and are unable to access our post regularly. There may be a delay in your registration if you complete a paper version.

A young carer is someone aged 5-17 years old – to complete your registation, hit the pink button below.

Professionals Referrals

As a professional you can either complete one of the above registration forms or complete the short referral form below by hitting the purple button. All forms require the consent of the carer.

The difference between the forms are:

  • The Young Carers and Adult Carers form requires all information completed within the form.
  • The Professionals form requires minimal information and we will need to contact the carer to ask them further information.
